WHO says new flu “unstoppable”, calls for vaccine

Saying the new H1N1 virus is “unstoppable”, the World Health Organization gave drug makers a full go-ahead to manufacture vaccines against the pandemic influenza strain on Monday and said healthcare workers should be the first to get one.

Every country will need to vaccinate citizens against the swine flu virus and must choose who else would get priority after nurses, doctors and technicians, said Dr. Marie-Paule Kieny, WHO director of the Initiative for Vaccine Research.

Several reports showed the new virus attacks people differently than seasonal flu — affecting younger people, the severely obese and seemingly healthy adults, and causing disease deep in the lungs.

Kieny briefed reporters on the findings of the WHO’s Strategic Advisory Group of Experts on Immunization, or SAGE. “The committee recognized that the H1N1 pandemic … is unstoppable and therefore that all countries need access to vaccine,” Kieney said.

“The SAGE recognized first that healthcare workers should be immunized in all countries in order to retain a functional health system as the virus evolves,” she added.

After that, each country should decide who is next in line, based on the virus’s unusual behavior.

Seasonal influenza is deadly enough — each year it is involved in 250,000 to 500,000 deaths globally. But most are the elderly or those with some kind of chronic disease that makes them more vulnerable to flu, such as asthma.

ELDERLY ADVANTAGE

The elderly seem to have some extra immunity to this new H1N1, which is a mixture of two swine viruses, one of which also contains genetic material from birds and humans. It is a very distant cousin of the H1N1 virus that caused the 1918 pandemic that killed 50 million to 100 million people.

A study published in the journal Nature on Monday confirmed that the blood of people born before 1920 carries antibodies to the 1918 strain, suggesting their immune systems remember a childhood infection.

The work by Dr. Yoshihiro Kawaoka also supports other studies that this new H1N1 strain does not stay in the nose and throat, as do most seasonal viruses.

“The H1N1 virus replicates significantly better in the lungs,” Kawaoka said. Other studies have also shown it can cause gastrointestinal effects, and that it targets people not usually thought of as being at high risk.

“Obesity has been observed to be one of the risk factors for more severe reaction to H1N1” — something never before seen, Kieny added. It is not clear if obese people may have undiagnosed health problems that make them susceptible, or if obesity in and of itself is a risk.

On Friday, a team at the U.S. Centers for Disease Control and Prevention and the University of Michigan reported that nine out of 10 patients treated in an intensive care unit there were obese. They also had unusual symptoms such as blood clots in the lungs and multiple organ failure.

None have recovered and three died.

The CDC estimates at least a million people are infected in the United States alone and clinics everywhere are advised not to test each and every patient, so keeping an accurate count of cases will be impossible. The United States has documented 211 deaths and WHO counted 429 early last week.

Kieny said WHO would also work to get better viruses for companies from which to make vaccines. She said the strains that had been distributed did not grow very well in chicken eggs — used to make all flu vaccines.

One exception — AstraZeneca’s MedImmune unit makes a live virus vaccine that is squirted up the nose and it is easier to produce, Kieny said.

WHO said countries should continue with their normal vaccination programs against seasonal flu. Kieny said the seasonal H3N2 strain was also very active now in the southern hemisphere’s winter.

Sanofi-Aventis, Novartis, Baxter, Schering-Plough’s Nobilon, GlaxoSmithKline, Solvay, CSL and AstraZeneca’s MedImmune are among those working on flu vaccines.

Cook’s lawyer, Orly Taitz, who has also challenged the legitimacy of Obama’s presidency in other courts, filed a request last week in federal court seeking a temporary restraining order and status as a conscientious objector for her client.

In the 20-page document — filed July 8 with the U.S. District Court for the Middle District of Georgia — the California-based Taitz asks the court to consider granting his client’s request based upon Cook’s belief that Obama is not a natural-born citizen of the United States and is therefore ineligible to serve as commander-in-chief of the U.S. Armed Forces.

Cook further states he “would be acting in violation of international law by engaging in military actions outside the United States under this President’s command. … simultaneously subjecting himself to possible prosecution as a war criminal by the faithful execution of these duties.”

The next time you apply for a credit card, your credit report and income will be only a part of the criteria used to determine your creditworthiness. For that matter, as long as you have the card, what you use it for will be noted and added to a growing set of data that makes up your psychological profile, which will then be referred to every time the bank deals with your or reevaluates your risk as a customer.

The New York Times Magazine takes a look at this new method of determining credit risk, pioneered by Canadian Tire executive J.P. Martin about 6 years ago.

Martin’s measurements were so precise that he could tell you the “riskiest” drinking establishment in Canada – Sharx Pool Bar in Montreal, where 47 percent of the patrons who used their Canadian Tire card missed four payments over 12 months. He could also tell you the “safest” products – premium birdseed and a device called a “snow roof rake” that homeowners use to remove high-up snowdrifts so they don’t fall on pedestrians.

It’s not just that what you buy reflects your socioeconomic level and current financial status, however; what Martin did was take the raw data and tease out personality traits that explained the the purchases while predicting future behavior.

Why did birdseed and snow-rake buyers pay off their debts? The answer, research indicated, was that those consumers felt a sense of responsibility toward the world, manifested in their spending on birds they didn’t own and pedestrians they might not know. Why were felt-pad buyers so upstanding? Because they wanted to protect their belongings, be they hardwood floors or credit scores. Why did chrome-skull owners skip out on their debts? “The person who buys a skull for their car, they are like people who go to a bar named Sharx,” Martin told me. “Would you give them a loan?”

Lenders have been using this sort of data mining ever since, but until recently they’ve kept it on the down-low to avoid triggering any privacy fears from customers. Now, with billions of dollars of losses from formerly profitable customers (i.e. the slightly risker ones) who suddenly can’t pay, the lenders are using their psychological data not only to screen for the “right” sorts of customers but also to try to convince the bad ones to pay off their debts.

There’s another reason for this, too: it helps build a stronger relationship with the customer.

If a credit-card company detects unsettling patterns, it might start cutting credit lines, raising interest rates or accelerating repayment schedules. (Companies are expected to withdraw $2.7 trillion of credit by the end of 2010, according to a March report from the Meredith Whitney Advisory Group, a banking-analyst firm.) But the most useful information the card companies are deriving from their data are the insights that help them deepen their relationships with customers, particularly when a cardholder is going through a rough time. One of the strongest conclusions of the psychological studies is that cardholders are most likely to pay the bills of those companies with which they have an emotional connection.

In the absence of my being there in New York City to stand with the 9/11 families, first responders and survivors, I offer the following statement in support of your goal of a new investigation into the attacks of September 11th and the NYC CAN campaign to place it on the ballot for November.

At the time of 9-11, I had been an FBI agent for over 20 years. My main responsibilities by then were teaching criminal procedure to FBI agents and other law enforcement officers, mostly about 4th Amendment search and seizure, 5th and 6th Amendment law of interrogation, right to attorney and constitutional protection of rights to “free speech”, due process, habeas corpus, and against cruel and unusual punishment. A week before 9-11, I and the rest of the FBI’s ethics instructors were mandated (as a result of an earlier public FBI scandal) to give a one hour PowerPoint presentation, a form of remedial training on “law enforcement ethics” which I accomplished in a fairly perfunctory way, just reading the slides.

After 9-11, with the knowledge I had of the bitter internal dispute inside the FBI that was being hushed up but had kept some of our better agents from possibly uncovering more of the 9-11 plot before it happened, I couldn’t forget two of the slides in that Law Enforcement ethics curriculum: “DO NOT: Puff, Shade, Tailor, Firm up, Stretch, Massage, or Tidy up statements of fact.” And “Misplaced Loyalties: As employees of the FBI, we must be aware that our highest loyalty is to the United States Constitution. We should never sacrifice the truth in order to obtain a desired result (e.g. conviction of a defendant) or to avoid personal or institutional embarrassment.”

The official dissembling and excuse-making about the true causes and prior mistakes that gave rise to and allowed the terrorist attacks to happen, almost immediately ushered in the Bush-Cheney Administration’s egregious and lawless, post 9-11 “war on terror” agenda which bore no connection to the original causes and no connection to the goal of reducing terrorism and making the world safer. When I got a chance, about 8 ½ months after 9-11 to tell what I knew, I did so and my disclosures led to further investigation by the Department of Justice Inspector General and figured in the 9-11 Commission Report.

But it was way too late for this emerging bit of truth that has continued to leak out in dribs and drabs to have any impact. The laws themselves, especially the criminal procedure ones rooted in the Constitution that I had spent my career teaching to law enforcement, have largely gone up in smoke. Having seen the cost of remaining silent, I publicly warned, a few months after my first memo, against launching the pre-emptive invasion of Iraq. But false agendas had already filled the vacuum created by lack of truth. And we are still dealing with the disastrous consequences of these unjustified, pre-emptive wars.

Let me therefore simply repeat the request I made to the Senate Judiciary Committee in June 2002: “Foremost, we owe it to the public, especially the victims of terrorism, to be completely honest. I can only imagine what these crime and terrorism victims continue to go through. They deserve nothing but the complete, unfettered truth.”

Therefore, I fully support the 9/11 families, first responders, survivors and over 60,000 other New Yorkers who have endorsed a new 9/11 investigation in New York City as advanced by ballot referendum this coming November election.
